• Students must complete work permit packet prior to issuance of work permit
• Allow up to two days for work permits to be processed
• Students must maintain a minimum GPA of a 2.0 with no “F” grades
• Excessive truancies may result in the revocation of your work permit

Work permit request form must be complete and signed by the student, parent, and employer. (Note: Employer must provide worker’s compensation carrier.) Please return completed form to the front office. If you have any questions, please contact the front office at 916-714-1653.
