EGUSD IEP/504 Students

8th and 10th Grade are currently impacted. You are welcomed to submit all the required paperwork and attend an orientation to be placed on a waiting list based on the lottery system.

ADMISSIONS REQUIREMENTS

Elk Grove Charter offers consultative RSP services for students on an Individualized Education Program (IEP). We ask students to submit their IEP or 504 plans to Elk Grove Charter for review during the enrollment process. Our special education and 504 departments will evaluate your plan to determine if placement at Charter (independent study program) is appropriate and would meet the student's needs. Please allow at least 5 business days for review.

To begin the review process, please obtain the following:

  1. Download and fill out an EGUSD Registration Form, located on the left side of this page.

    • All fields must be completed

    • ALL individuals on the birth certificate must appear in the Parent/Guardian section of the registration form. If those individuals no longer have educational rights, a court order showing so must be provided.

    • Step-parents should be listed under emergency contacts. If the court system has granted them educational rights, a court order showing so must be provided at orientation.

  2. Alternative Education Referral (from student's referring school) 

  3. A copy of your students most recent transcript (9-12) or report card (7-8)

  4. Attendance printout

  5. Discipline summary

  6. Immunization record showing that the student has received the sixth TdAP booster - (GRADE 7 ONLY)

  7. Birth Certificate

    • If you cannot locate a copy of the birth certificate, please click here to find out how to contact the county clerk/recorder where you may order another copy

  8. Proof of Residence (SMUD/PG&E bill or lease agreement ONLY)

  9. Alternative Education Referral (GRADES 9-12 ONLY)

  10. The IEP/504 team at the current school of enrollment must meet and have a discussion to determine if a change of placement would be appropriate and that placement at Elk Grove Charter and into an Independent Study environment would meet the student's needs. If that determination is made, a copy of the IEP/504 must be provided with a signed addendum indicating their determination.

Once you have obtained these items, please bring them to our office between the hours of 8:00am and 4:00pm.

Do not dis-enroll your student from their current school! Your student is not enrolled at Elk Grove Charter until the orientation process has been completed.

Please note: Alternative Education referrals and any other paperwork must be presented in person to our office. We will not accept any paperwork via fax or U.S. mail. With the exception of original birth certificates and proofs of address, all submitted application paperwork becomes property of the Elk Grove Charter School. If you require personal copies, please make them before submission. We are unable to process admissions requests that have missing or incomplete information.

 

ADDITIONAL IEP INFORMATION

At Elk Grove Charter School, there is one full-time, on-site Resource Specialist responsible to serve as caseload manager for all students on an Individualized Education Program (IEP). The Resource Teacher works in collaboration with the general education teachers in implementing the individualized student accommodations in the general education curriculum/setting.

Offer of FAPE (Free and Appropriate Public Education) at Elk Grove Charter School: Parents are advised prior to enrollment that Elk Grove Charter School is a parent's choice school that offers an independent study program (ISP) as an alternative model of education with consultative special education services. If student's special education needs are greater than what can be met in this model, the full continuum of services may be available through the family's comprehensive school of residence.
